I have read and understand the rules for the Texas Firefighter
Olympics Bass Tournament event. I have completed this application and have
included my entry fee to participate. I hereby agree to be bound by and comply
with all tournament, federal, and state rules and regulations. I expressly
assume all risk(s) associated with each tournament event and I hereby release
the Texas Firefighter Olympics, the host, all sponsors, and tournament officials
from all claims or injury and/or damage incurred in connection with any Texas
Firefighter Olympics sponsored event(s). I hereby grant Texas Firefighter
Olympics the unconditional right to use my name, voice, & photographic
likeness in connection with Texas Firefighter Olympics promotions and/or
articles and press releases. I shall not be entitled to receive any royalties or
other compensation in connection with such use. I further understand and agree
that the tournament director reserves the right to reject my application for any
reason. I have sufficient health, accident, and liability insurance to cover any
bodily injury or property damage incurred by myself or others as a result of my
participation in any Texas Firefighter Olympics sponsored event. If I have no
such insurance, I declare that I am capable of paying for any and all such
expenses or liability. I understand that I am responsible for paying all local,
state, and federal taxes, title, license and registration as a result of my
participation in any Texas Firefighter Olympics sponsored event. By signing this
release, I am stating that I agree to these terms.

Pre-entry Fee is $50.00 for the 2 day event. This includes big bass for
both days. Payout will be 1 place for every 5 entries. 80% payback. Medals will
be awarded for 1st, 2nd, and 3rd place combined
stringer weight for both days, and first place big bass for both days.
Please only send a check, cashier’s check, or money order in the mail.

Late entry will begin 1 hour before launch on tournament morning. Late
entry fee is $60 cash at the lake. EXACT CHANGE ONLY!!!
An angler may fish only 1 day of the tourney if they cannot make both days,
but will still pay the full entry, and will use their one day stringer weight as
the combined stringer weight for the tourney.
